Please note that the below forms need to be signed and returned to the church office. Once they have been reviewed by the Board of Trustees the submitter will be informed of any decision. Completion and submission of a form below is not equal to a reservation or commitment on the part of the United Methodist Church of Hyde Park. All submissions must be reviewed and the result of that review communicated to the submitter before any use of the building is to be considered scheduled by the Church Office.
